Who is the program for?

It is for individuals across Ontario who are living with one or more chronic conditions such as diabetes, arthritis, respiratory disease, kidney disease, chronic pain, cancer, or any type of long-term physical or mental health issue. Family, friends, and caregivers of someone who has a chronic condition are also welcome. Individuals looking to improve their health literacy will also greatly benefit from our programs.

Is the program suitable for everyone?

Our programs are interactive and asks participants to set goals and participate in discussions, so the program may not be suitable for individuals not willing or able to participate in this way. For instance, the program may not be appropriate for someone with dementia or memory impairment. However in these cases family members or friends may benefit by learning self-management skills in order to help support others and take care of themselves.

What does it cost to participate?

Participation is FREE.

Do I need to be referred to the program?

No. While some people are referred, anyone who is interested is welcome to participate.

How many people are in a workshop?

There are about 10 individuals participating together in one Help With Your Health workshop. This allows for dynamic discussion and interaction among the group of participants. Health Coaching is a one-one program.

Does this program replace existing programs?

The online workshops and Health Coaching do not conflict with existing treatments or programs, and is designed to enhance disease-specific education. The program may be especially helpful to individuals who are living with more than one chronic condition, providing them with the skills and confidence to better manage their health.

Can I access the programs from a tablet or mobile device?

The programs can be accessed from a mobile device or tablet.
